The attached registration packet(this will be handed out at registration time) is an important part of the registration
process for our schools. This information allows us to do the best possible job
in meeting your child’s needs. Thank you for taking the time to complete this
information in a thorough fashion.
The following items MUST BE BROUGHT to school when
registering your child:

bill, certificate of occupancy, government/social agency ID showing address

Rocky Hill Residency form completed and notarized (only for students residing
with someone other than parents or families living in temporary residence)

including (original or copy obtained from previous school or physician)
q Physical Examination as mandated by the State of

height, weight,
blood pressure, hematocrit/hemoglobin,
postural, vision and
hearing screenings
q Proof of
Immunization as mandated by the State of

Tuberculosis or documented negative risk assessment

q Academic records (including number of high school
